**Steps to Book Your Appointment:**
The third parties listed below manage the scheduling of the appointments:

- Headway
- Zocdoc
Click the "Schedule Appointment" button to schedule and book an appointment with us.
- Choose an appointment date.
- Select a time for your appointment. (All times are in Eastern Time - New York.)
- Once you schedule an appointment, we will respond to your enquiry within 24 hours. However, you can schedule an appointment online, 24 hours a day, 7 days a week.
To complete the booking process:
- Once your appointment is confirmed, you will need to complete the new patient package and submit it online.
- Your signature is needed to give your consent.
- The package (or "intake forms") includes a consent form for a telehealth session and requires you to provide your medical history, insurance card, and a positive ID, such as your driver's license.
Video conference:
- After you book an appointment, you will receive a video conference link and further details in the booking confirmation emails (including intake forms).
